
ECHO Diabetes
Through my internship I assist in a diabetes care support program for primary care providers serving patients with Type 1 and insulin-dependent Type 2 diabetes in targeted clinical settings.
We also support hub-to-provider tele-education, coordinated multidisciplinary clinical assistance, and facilitated connections between clinics, diabetes specialists, and patient support services.

Why ECHO Diabetes
Project ECHO Diabetes uses the ECHO Model™ to support primary care providers serving adult and pediatric patients with Type 1 or insulin-dependent Type 2 diabetes in underserved communities with limited access to specialty care. Through partnerships with PCPs and FQHCs, the program delivers evidence-based education, real-time specialty support, and assigns trained Diabetes Support Coaches, peers living with T1D, to help address care barriers and improve access to high-quality diabetes management.
